Offers education to member about Case Management services and available resources in coordination with Case Managers.

Participates in physician/case management/concurrent review rounds as needed.

Creates episode for referral to Case Management.

3-5 years of related managed care experience required

Verifies benefit details with the health plans.

Working knowledge of medical terminology

Experience of Transition Lifecycle Management, including service desk, Cloud & Data Centers, End User Computing, Network management, hardware maintenance etc.

Risk management experience with the ability to proactively assess/manage/mitigate risks.

Very good planning skills with proficiency in using project management tools like MS projects.

Knowledge and experience of People Transfer methods, requirements, and legislation specific to the Transition scope.

Escalate major Transition issues to Management with recommendations on how to address.

Knowledge of Outsourced Managed Services Delivery methods, practices, processes and tools.

Assist clients in the use of automated support systems and tools (e.g. DD Form 2648 eForm for CAPSTONE eBenefits, MyArmyBenefits.com, etc.).

Comply with the instructions of the Contract Installation Manager (CIM) to perform tasks.

Strong verbal/written communication skills along with needs assessment, individual/small group training.

Employability skills, job search strategies and placement techniques

Relies on experience and judgment to plan and accomplish goals. A certain degree of creativity and latitude is required.

Demonstrated experience in conducting program evaluations and use of customer feedback to improve overall program effectiveness/impact highly desired.
