MSW/DSW/PHD in Social Work from a school accredited by the Council on Social Work Education (CSWE)

Prior experience with mentally and /or chemically dependent population required

Experience in advocacy, case management, discharge planning, individual, inpatient, resource entitlements, short-term crisis preferred.

1 year Post-masters work experience in a Physical Disabilities setting, preferred

6 Months Post-masters work experience with an Adult population, required

1 Year Post-masters work experience with an Aged population, required

1 Year Post-masters work experience in an Abuse/Neglect/Domestic Violence setting, preferred

1 Year Post-masters work experience in an Acute/ Chronic/Progressive Illness setting, preferred

Consults with program management regarding students training program, progress, development, and placement experience; meets with students faculty advisor at prescribed intervals

Provides psycho education, counseling, and crisis intervention to patients and their families

Incorporates knowledge of community resource sand entitlement programs in all planning

1-3 years of relevant inpatient behavioral health experience required or substantial experience in outpatient

Provides interdisciplinary team with assessment findings and suggests potential impact on overall care of patient

Maintains appropriate professional patient care documentation as defined by site policies and procedures

Previous supervisory experience and strong clinical and assessment skills

Manage staff and promote a staff culture that emphasizes housing placement as a measurement of success

Minimum 3 years experience working with chronically homeless populations

Provide clinical direction and leadership to the program; Help troubleshoot, problem-solve solutions around client engagement and challenging behaviors

Ensure that clients are ready for housing placement within 90 days of entering the Drop-In Center

Oversee development & implementation of individual service plans; ensure documentation of client contact and progress is timely

Collaborate with inter-disciplinary team in providing patient care.

Provide reports, record documentation and statistics as required.

May serve as liaison within the department, hospital and to community agencies.

Perform other activities as shall be deemed appropriate in assuring services to patients and the functioning of the Social Work Program.

2+ years of experience providing social work or mental health services to youth and young adults

1+ years of experience working with disconnected youth and young adults within the past 5 years
