Social Worker Hospice Jobs in Alaska

Visits hospice patients and families to complete psychosocial assessment provide education and support.

Counsels and assists patients and their families to participate in the development of the hospice plan of care.

Serves as supportive caregiver to the Hospice Interdisciplinary Team.

Responsible for the development, implementation and timely revision of the care plan in coordination with the family and hospice team.

Participates in interdisciplinary team meetings providing Psychosocial assessment information relevant to the plan of care.

Is available for emergency on-call service.
