Strong planning and organizational skills.
CPR certification through the American Heart Association is required.
Required Licenses, Registrations And/or Certifications
Performs (or evaluates) initial assessment to determine social service needs of patient on physician referral.
Provides (or assists in providing) direct social service interventions, including coordinating care plan interventions with other community agencies.

They also work with health care providers to ensure that individuals receive the best possible care. They may also provide crisis intervention and support to individuals and families in times of need. Job Skills Required
