Current Oncology Nurse Certification in New York State required.

Reviews all protocols to maintain quality control and patient safety, and to assist Physicians with patient’s compliance.

Participates in quality meetings to discuss cases and review clinical outcomes.

You have 1+ years experience in case management

Deliver coordinated, patient-centered virtual Care Management by telephone that improves members' health outcomes.

Lead outreach efforts to high-risk individuals and complete care management assessments and care plans for patients enrolled in the program

Work with other disciplines on co-managed cases and collaborate with other teams to assist with benefits and claims questions

You are self-aware, able to prioritize and manage multiple assignments, while maintaining department performance standards in a remote setting

Help members navigate the complexity of the healthcare system, providing education and empowerment to support members in meeting their healthcare goals

Experience with clinical quality, quality improvement, and/or risk management;

Detail oriented and highly organized with effective time management skills.

2 years of Behavioral Health experience

2 years of Med Surge experience

Clinical quality experience is a plus

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

Budget management, contract development, contract monitoring, continuous quality improvement.

To be considered for this position, applicants must meet the minimum qualifications for a Registered Nurse Specialist/Supervisor.

Develop and adhere to policies/procedures, standards, and contract monitoring tools.

Collaborate with other chronic disease programs to focus on outcomes through identifying data needs, and evaluating impacts.

Provide oversight and assist as needed in the development of grant applications and various grant reports, and ensure timely submission.

2 years of Behavioral Health and Med Surge experience a must

2 years clinical experience essential

2-3 year critical care experience (which includes ER experience or Cath Lab experience).

Graduate of an accredited School of Nursing.

Valid license as Registered Nurse in Connecticut.

Maintain minimum of 6 contact hours annually.

Participate in educational sessions for professionals, end users and interest groups.

Ability to manage difficult situations effectively

Is able to work under pressure and prioritise workload to ensure that the service offered meets the expectation of the customer.

Has specialist knowledge of ConvaTec product and the Amcare Group home delivery service, and advises end users on appropriate choice.

High Level of organisational skills and an ability to prioritise workload

Excellent communication and networking skills. Able to Communicate sensitive, condition related information to patients and relatives

Salary: $2035.36 - 2454.25 (depends on years of experience)

At the time of this advertisement shift differential is $3/hourly (subject to change)
