As a Home Health Quality Coordinator you need to know how to:

Perform quality assessment of clinical records including diagnosis coding, review of assessment and plans of care.

Collaborate with leadership to develop quality indicators and QAPI initiatives for agency.

Team up with associates to ensure that services are provided at quality levels consistent with professional standards and goals.

Operations, leadership, critical thinking, analysis, teaching, facilitation, coaching, mentoring and project management skills required

Knowledge of population health management practices, policy, programs, initiatives and functions.

Course work in Quality Management, Regulatory Compliance, and Statistics preferred.

Knowledge of and experience in process improvement (e.g., lean or process transformation).

8 years recent relevant experience

Exemplary written and oral communication skills; to include presentations and comprehensive analysis reporting on performance improvement issues.

Quality/ Risk/Patient Safety/Just Culture/Project Management/Lean/IP certification.

Assists with quality management studies through data collection, data input and report development.

Analyzes risk management/patient safety principles and provides recommendations for the incorporation of risk management principles in each medical center.

Provides support services to appropriate medical center staff in implementing risk management principles into credentialing processes.

Must be able to work in a Labor/Management Partnership environment.

Minimum of three (3) years' experience working in quality operations in an acute care setting.

Medical diagnoses, procedures, and acceptable treatment patterns. Inpatient hospital experience preferred.

Training and educational development for adult learners.

Plan, perform, and evaluate educational opportunities through a variety of means.

Utilization review, quality, and peer review processes/procedures.

And ability to utilize personal computer and corresponding applications including Microsoft Office (Word, Excel, Access, PowerPoint).

Organizing workload and preparation of correspondence for provider community and physician consultant reviewers.

One-three (1-3) years clinical experience including general knowledge of QM principles, NYPORTS and JCAHO requirements.

Knowledgeable of Performance Improvement methodology used within the organization.

Assists in coordinating investigations of state reportable incidents/complaints, sentinel events, citations and data collection for corrective actions.

Uses office technology and data base systems to perform functions of the job; creates documents for formal presentations and storyboards.

Licensed as a Registered Nurse in the state of South Dakota and/or meeting the requirements of multi-state licensure required.

Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.

Bachelor's degree in nursing preferred.

80 hours per 2 weeks

$29.50 - $44.25 per hour

Computer proficiency in Microsoft Office and Internet.

Thorough knowledge of health care delivery and quality improvement is required, including clinical care management and/or quality management.

Knowledge of lean performance improvement methodology and management systems.

Demonstrated skills in consulting, group process facilitation, whole system design, negotiation, change management and conflict resolution.

Skilled in teaching improvement concepts and principles to diverse audiences from senior management to front line staff.

Specific training/experience in project management

Bachelor's: Nursing or related field or equivalent education/experience

Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.

Collects and collates data from patient medical records and enters it into a registry form.

Communicates and interacts with physicians and patients to ensure all information is obtained accurately and completely.

Monitors the process of data collection to ensure time frames are met.

Generates reports from the registry and analyzes data to compare organization to similar institutions regarding to key quality indicators.

Measures patient, provider and facility characteristics, cardiac device type and adverse events.
