Accurately collect data needed for the appropriate portion of the integrated assessment.

Take and record vital signs properly.

Observe for, and document changes in patient’s appearance, behavior and ability to work toward treatment plan goals.

Communicate changes in patient’s condition to the Charge Nurse.

Report signs and symptoms of infection in resident to the Charge Nurse

Qualifications /Training: Zero to less than 1 year of work related experience

Licenses/Certifications/Registrations/Education: HS diploma / GED equivalent preferred

Initiates and maintains the hygiene/ cleanliness of the patients, the patient’s environment, and the unit environment.

Using Service Excellence Standards to conduct patient safety rounds as indicated by level of acuity
