Has real-time knowledge of capacity and capabilities of MGB Home Hospital

Clinical knowledge and skill in real-time evaluation of patients during their Emergency Department encounter

Communicates and collaborates with Administrative Coordinator and Administrative Manager regarding patient transfers

Two (2) years of nursing experience

Must have current healthcare provider BLS for Healthcare Providers certification from one of the following:

Must be able to demonstrate a working knowledge of laws and regulations governing Medicare, Medicaid and other funding sources.

Must be a graduate from an accredited school of nursing.

Must have current, valid RN license or temporary RN license from the Texas Board of Nursing; or, valid Compact RN license.

Must be self directed and capable of priority setting and problem solving.

5. Excellent time management skills needed to prioritize competing priorities.

3. Current Basic Life Support (BLS) Certification.

2. Provides communication, advocacy, and education in a culturally competent manner.

6. Collects and reports data from patient on barriers, hospital experience, health care, medications, prior treatments, etc.

4. Knowledge of and application of the nursing process.

1. Bachelor of Science in Nursing (BSN).

Educate patients about their healthcare, insurance benefits, and common medical conditions

You are a self-starter and are comfortable with a remote working environment

You can travel in the community to meet patients at home, work, or at their doctor visits

Develop strong relationships with patients to assist them with their care

Work collaboratively with other team members to ensure patients receive exceptional service and care

Help patients with medication adherence via reminders and best practices
