Outreach Instructor Jobs

Effectively manages the clinical operations of two satellite counseling and educational spaces.

Provides time-limited individual and group psychotherapeutic treatment, psychological evaluations, and urgent care and crisis intervention within satellite counseling and educational spaces.

Evidence of experience in developing and managing new clinical initiatives or systems of care

Specialized knowledge and expertise in working with students who hold (multiple) marginalized identities, particularly those who identify as LGBTQIA+ and BIPOC

Experience working in a university mental health service or a comparable healthcare system

Serves on an on call-after hours rotation with other CPS staff.

Experience working with people in mental health recovery.

Starting $17.00/hr - $1.00 wage differential for Medication Administration Program (MAP) certification.

Assist participants residing in a variety of settings to attain their individualized recovery goals and objectives.

Minimum 1-2 years of experience in job-related duties

Working Conditions, Environment and Physical Requirements:

Prepares assignments and lesson plans for classes as directed by lead instructor

Management Program for appointments and services coordination as needed.

Implements a comprehensive clinical care management plan for each patient.

Maintain a complete and precise documentation of all interventions performed in the Care Management documentation tool.

Collaborates in a patient care process to assess, plan, facilitates, coordinate, monitor and evaluate options and services that meets member’s needs.

Identifies special needs of members, and refers cases to the interdisciplinary care team, as necessary.

Perform quality checks and report trends to leadership and during interdisciplinary care team meetings accordingly.

Experience working with people in mental health recovery.

Facilitate a warm and welcoming Family Engagement Center

Build positive relationships with the community, parents, students, teachers, staff, administrators, and co-workers.

Parent involvement in school- gradebook, portal, etc…

Attend District professional development trainings.

Assist principals and school community councils with partnership activities that meet the goals of their school parent involvement plan.

Provide harm reduction and health education.

Street Outreach will be conducted in at least teams of two in identified high risk areas.

Outreach will be provided to service providers in the area to introduce SAP and ARCH services.

Actively seek and identify individuals or groups requiring the agency's services.

Conduct an assessment of need, which includes type of drug use and number of injections daily/weekly.

Complete and maintain data collection as required.

Provide follow up interventions including phone calls and escorts to participants in need of case management and supportive services.

Attends and participates in continuing educational programs, seminars, etc., designated to maintain professional competence.

Participates in outreach/field events as scheduled.

Maintain up to date client records in accordance with grant and HIPPA protocols.

Promote YAP program through various outlets including internet/social media, in-reach and out-reach activities.

Provide testing on a daily bases as clients must complete DOH HIV testing training.
