Overview A Nurse Case Manager is a registered nurse who is responsible for providing comprehensive care management services to patients. Detailed Job Description Nurse Case Managers are responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the options and services required to meet the patient’s health needs. They work with other healthcare professionals to ensure that the patient receives the best possible care. They also provide education and support to the patient and their family. Nurse Case Managers are responsible for developing and implementing care plans for their patients. They must be able to assess the patient’s needs and develop a plan of care that meets those needs. They must also be able to monitor the patient’s progress and make adjustments to the plan of care as needed. Nurse Case Managers must also be able to communicate effectively with other healthcare professionals, patients, and their families. They must be able to explain medical terminology and procedures in a way that is easy to understand. They must also be able to provide emotional support to patients and their families.
