Employ listening skills in order to understand all of the caller's needs.

Identify and offer effective solutions to both expressed and latent needs.

Connect callers with resources in the caller's area that offer assistance.

Utilize the department's database information, demonstrating technical skills that lead to efficient and effective outcomes.

High school diploma, with some social services coursework or experience preferred

A minimum of 1 year of experience in a high-volume crisis center

Collects, tracks and reports information about the community and community benefits.

Provides care, support, follow up, and education in community settings.

Provides culturally and linguistically appropriate health education.

Effective oral and written communication skills.

Personal knowledge of the target population, as shown by residency in that neighborhood.

Knowledge of local resources and system navigation.

Should be highly skilled in: leadership or coaching, conflict resolution, time management, cooperation/teamwork, developing positive professional relationships.

Two years experience providing direct services to persons with developmental disabilities or in community outreach or other social service settings.

Communicate with family members, care providers, case managers

Bachelor's Degree preferred but can substitute for experience in relevant field.

Strong written and verbal skills.

One year of continuous, verifiable experience in a relevant field such as rehabilitation, retail, production, or food service.

High School Diploma and 3 years of experience in social and/or community services or equivalent combination of education and experience.

Experience and knowledge with lesson planning and clerical/administrative tasks.

Conduct educational and informational workshops on agency grounds and at Community Partner Locations.

Maintain appropriate credentials and certifications necessary to provide essential client services.

Excellent communication skills as required for effective interpersonal communication with clients.

Conduct effective and thorough client outreach throughout the community in order to recruit eligible clients.

Identify opportunities for partnering with community, provider, and advocacy organizations to establish stronger and visible relationships with various constituencies

Monitor and report on achievement of committed action plans

Facilitate and maintain communication between internal and external audiences Coordinate outreach activities to identify enrollees needing care within various populations

Assist in coordinating and tracking funding and sponsorship requests in partnership with marketing team

Manage HCS requests for Supportive Supervision benefits within identified timeframes

Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients

Experience in a managed care environment

Utilize advanced clinical skills to review Care Assessments and available collateral information against criteria to support decision making

Manage Behavioral Health Personal Care authorization process per protocol

Partner with finance and local leadership to provide necessary reporting and reconciliation as necessary

Seeks consultation and educational opportunities to improve skills.

Development of employment skills that you can take with you anywhere: teamwork, problem solving, adaptability, communication, leadership, creativity

Alert Program Manager and LE of major incidents or emergencies, following protocol.

Demonstrate experience working with the mental health system, NAMI, or other coalitions.

Must have broad, working knowledge of area resources and ability to identify and connect people to appropriate resources.

Demonstrates knowledge of court orders and the criminal justice system.
