Med Care Manager Jobs in Indiana

Provide training with service providers on Home Care program operation and care management functions.

Provide advocacy and assistance for consumers in securing benefits and services when consumer or others are unable to do so.

Participate in in-service training, courses and workshops to maintain professional expertise and develop additional skills.

Assessment of an older adult's physical, social, emotional and environmental status to determine their eligibility and needs.

Development and implementation of a service plan to address those needs by making referrals to vendors and other community agencies.

Arrangement for crisis intervention services when necessary.

Completes a care-management progress note corresponding with each on-site visit or encounter and upon significant change

Proficiency in common software and web-based applications (e.g. Case Management Systems, Office)

Additional state-specific qualifications may be required.

Completes initial home assessments, collects required caregiver credentialing documents and ensures that all required home modifications are completed

Obtains a social history and conducts a psychosocial evaluation

Assists with obtaining information about and making referrals to, other health-care and community services
