Master Medical Social Worker Jobs in Utah

Must be willing to work in a Labor Management Partnership environment.

Coordinates exchange of information between Kaiser, families, members and skilled nursing facilities.

Determines application of Kaiser, Medicare and Medi-Cal benefits to specific patient situations.

Step I: 0 - 2 years social work experience.

Step II: 2 - 4 years social work experience within the last five (5) years.

Step III: 4 or more years social work experience within the last ten (10) years.

Experience working in clinical care management, social work or with third party payers.

Basic computer skills and knowledge of word processing and spreadsheet software.

Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified.

Strong written and verbal communication skills.

Knowledge of health insurance related products.

2 years of post Master's experience in a health care setting, preferred

Strong planning and organizational skills.

Required Licenses, Registrations And/or Certifications

Performs (or evaluates) initial assessment to determine social service needs of patient on physician referral.

Provides (or assists in providing) direct social service interventions, including coordinating care plan interventions with other community agencies.

Participates in the interdisciplinary team for the development and revisions of the Plan of Care.
