Provides case management, including enhancement of adult daily living skills such as self-care, nutrition, and sanitary skills
Facilitates relationships between resident and landlord/property manager, and advocates for residents when necessary.
Supports and encourages resident participation in educational or vocational programs.
This position is responsible for coordinating services for recovery, employment, education, support groups, and housing.
Assists residents in obtaining housing that meets standards of quality set by HUD
Visits apartments at least twice a month with actual time frames determined by resident needs.

Secure information such as medical, psychological, and social factors contributing to client’s situation, and evaluate these and client’s capacities.
Refer clients to community resources and other organizations. Network with community service providers.
Complete all necessary reporting forms for government agencies and funding sources (UCR’s, progress notes, referrals, and statistics). Maintain client case files.
Maintain appropriate communication with supervisors and other funding sources. Notify of any safety issues in a timely manner.
