Experience-Two years experience working with adult/geriatric patients in a hospital or healthcare clinic setting preferred.
Performs psychosocial assessment of hospice patients according to regulatory and agency time frames & patient care plan.
Participates in developing interdisciplinary care for the hospice patients by participating in weekly IDT meeting.
Recruit; train & supervise Hospice volunteers.
Work with Hospice team to develop care plan to meet individualized patient/caregiver psychosocial needs.
Document patient interactions/visits according to agency policy
