Home Health Rn Care Coordinator Jobs in Alabama

Provides supervision of LPNs and home health aides per regulatory requirements.

Responsible for the productivity of a full time RN Care Manager.

Performs compliance requirements as outlined in the Employee Handbook

Uses health assessment data to determine nursing diagnosis.

Counsels and provides instruction to the patient and family in meeting nursing and related needs.

Presents patients at IDT patient conferences.

Knowledge of behavior management, crisis intervention and, interventions common to acute psychotic residents as well as to non-violent crisis intervention practice

Ongoing compliance with Nurse Delegation Program, including recertification requirements, taking MAS call, and conducting MAC worker check off and remediation.

Knowledge of educational methods appropriate to adults with severe and persistent psychiatric disorders

Manages incident files in RL for designated programs to ensure policy and protocol is followed.

Actively works to enhance clinical skills

Maintains current state license and requirements for renewals (i.e. CEU’s), and attends the required in-services and workshops
