Overview A Home Health Admission Nurse is responsible for assessing, planning, and coordinating the care of patients in home health care settings. They provide direct patient care, coordinate with other healthcare professionals, and ensure that the patient’s needs are met. Detailed Job Description Home Health Admission Nurses are responsible for assessing the needs of patients in home health care settings. They must assess the patient’s physical, mental, and emotional condition, and develop a plan of care that meets the patient’s needs. They must also coordinate with other healthcare professionals, such as physicians, physical therapists, and social workers, to ensure that the patient’s needs are met. Home Health Admission Nurses must also provide direct patient care, such as administering medications, providing wound care, and providing emotional support. Job Skills Required
