Develop initial assessment and Co-Occurring contract with incoming persons served. Review co-occurring contract annually or as needed.
Complete contact notes in EHR, Individual Service Plans, Monthly Progress Notes, annual documents, and other documents as needed and/or assigned.
Assist with locating and attending self-help groups (AA/NA, etc.).

Detailed Job Description The FSP Case Manager is responsible for providing case management services to families and individuals who are receiving financial assistance from the Family Support Program. The Case Manager will work closely with the family or individual to identify and address any barriers to success, and will provide resources and referrals to help the family or individual achieve their goals.
