Detailed Job Description:

The Discharge Planning Coordinator is responsible for assessing the patient’s needs and developing a discharge plan that meets the patient’s needs. This includes assessing the patient’s physical, psychological, and social needs, as well as the patient’s financial resources. The Discharge Planning Coordinator will also coordinate with other healthcare professionals, such as physicians, nurses, and social workers, to ensure that the patient’s needs are met. The Discharge Planning Coordinator will also coordinate with community resources, such as home health agencies, to ensure that the patient is provided with the necessary resources to transition to the next level of care.
