Manage both medical and behavioral, chronic and acute conditions effectively, and in collaboration with a physician or specialty provider

Understand the Payer/Plan benefits, CCM associate policies, procedures and articulate them effectively to providers, members and key decision-makers

1+ years of hands-on post grad experience within Long Term Care

Proficient computer skills including the ability to document medical information with written and electronic medical records

Deliver cost-effective, quality care to assigned members

Speaks knowledgeably about Option Care’s scope of services and effectively instructs patients about related financial obligations for care and service charges.

Participates in nursing department on-call responsibilities.

Actively participates in facility process improvement program and assists Supervisor or Nurse Manager with chart audit functions as applicable.

Safety requirements include closed toe-shoes, facemask, goggles, gown and/or gloves and functioning device for communication in both routine and emergency situations.

Preferred Qualifications & Interests (PQIs)

Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards.

Certification or Registration as a Medical Assistant upon hire and maintained during employment.

Receive appropriate documentation and implement physicians verbal and/or telephone orders.

Assess individual residents to determine ability for self-administration of medication, provide oversight of medication storage and administration.

Provide support to admission staff as needed to conduct prospective resident interview and assessments.

Utilize established assessment process to determine level of wellness in conjunction with LPN Charge Nurses, resident, family, and influencers.

Provide supportive oversight of identification of a DPOA and establishment of end-of-life care preferences.

Obtains referrals for other skilled services as indicated.

Provides emotional support and education to patients and their families.

Follows all Hospice policies and procedures and ensures compliance with all federal and state laws, regulations and JCAHO requirements.

Completes all required professional education including continuing nursing education hours and mandatory in-services to maintain professional licensure.

Performs other related job duties as assigned.

Assesses patient and develops plan of care, establishes mutually agreed upon goals with the patient, caregivers, and/or family members.
