Detailed Job Description A Dialysis Social Worker is responsible for providing psychosocial support to patients with ESRD and their families. This includes providing counseling, education, and advocacy to help patients and their families cope with the physical, emotional, and financial challenges of ESRD. They also provide referrals to other healthcare professionals and community resources, and coordinate care with other healthcare providers. Job Skills Required
• Knowledge of ESRD and its treatments
• Knowledge of psychosocial issues related to ESRD
• Ability to provide counseling and education
• Ability to advocate for patients and their families
• Ability to coordinate care with other healthcare providers
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
Job Qualifications
• Bachelor’s degree in social work or a related field
• Master’s degree in social work or a related field preferred
• Licensure as a social worker in the state of practice
• Certification in dialysis social work preferred
Job Knowledge
• Knowledge of ESRD and its treatments
• Knowledge of psychosocial issues related to ESRD
• Knowledge of community resources and referral processes
Job Experience
• Previous experience working with patients with ESRD preferred
• Previous experience working in a healthcare setting preferred
Job Responsibilities

• Refer patients and their families to other healthcare professionals and community resources

• Monitor patient progress and provide follow-up care as needed
