Prioritizes care based on patient’s current needs. Proactive in preventing adverse outcomes. Demonstrates follow-through in the completion of patient care.
Initiates and/or reviews appropriate plan of care with patient/family to address holistic age-specific needs and reflect the patient’s changing condition.
Consistently documents the assessment, planning, interventions and patient responses in a way that accurately reflects the patient’s care and progress.
Clinical Nurse - Wound Care

Assist in the development, coordination, and implementation of Plans of Care.
