Collaborates and facilitates patient discharge plans for those requiring post Hospice or post inpatient care.
Assists patient/family/caregiver in determining appropriate level of Hospice care. Refers to outpatient resources as appropriate.

They assess the needs of their clients, develop individualized plans to address those needs, and provide resources and support to help them achieve their goals. They also provide counseling, crisis intervention, and advocacy services. They may also provide referrals to other social services, such as housing, employment, and health care. Job Skills Required

Job Qualifications
• Bachelor’s degree in social work or a related field

• Licensure as a social worker in the state of practice
