Requires possession of a valid Florida Drivers License.

Current BLS from the American Heart Association or American Red Cross.

1+ years of administration support experience in a member facing healthcare environment

Working knowledge using MS Office Word, Excel, Outlook, Excel, Shared Systems, keyboard, web navigation, and troubleshoot and resolve general technical issues

Facilitate distribution of Harm Reduction supplies according to evidence – base practices with a focus on safe use.

Conduct outreach to engage individuals in harm reduction services.

Develop and maintain therapeutic relationships with participants utilizing Motivational Interviewing techniques.

Assist participants in identifying personal recovery goals and setting objectives for each goal.

Assist participants in problem solving and maintaining self-care needs. Assist participants in establishing self-sufficiency, continuing treatment gains.

High School Diploma/GED (or higher) or 4+ years of case management experience

Collaborate with Care Managers, providers, and community agencies and organizations to identify consumers who may benefit from peer support

Completion of Stanford Chronic Disease Self-Management Program

Completion of Whole Health Action Management Training

Experience working with consumers that have co-occurring Disorder, i.e., Mental Health and Substance Use Disorder

Support the creation and assist with the implementation of a comprehensive Training and Education program with peers, families, providers, and staff
