Detailed Job Description A Care Delivery Specialist is responsible for providing quality care and support to patients and their families. They are responsible for managing the care of patients, coordinating care plans, and providing education and support to patients and their families. They must be able to assess patient needs, develop and implement care plans, and provide emotional support to patients and their families. They must also be able to communicate effectively with other healthcare professionals, coordinate care with other providers, and ensure that the patient's care is in compliance with all applicable laws and regulations. • Excellent communication and interpersonal skills
• Ability to assess patient needs and develop care plans

Job Qualifications
• Bachelor’s degree in nursing, social work, or a related field
