REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

Full-time employment, 40-hour work week.

Responsible for case management/discharge planning.

Handle with Care Certification required with 6 months of hire or transfer into the position.

Completes Psychosocial Assessments within 72 hours of admission.

Responsible for completing and updating the BH Master Treatment Plan.

Conducts group, individual and family therapies.

Assesses patient's progress and documents appropriately.

Develop and implement treatment plans based on clinical experience and knowledge

Act as client advocate in order to coordinate required services or to resolve emergency problems in crisis situations

Collect information about clients through interviews, observation, and tests

Counsel clients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life

Evaluate clients' physical or mental condition based on review of client information

Maintain confidentiality of records relating to client's treatment

Bachelor’s Degree in a related field AND 2 years of case management experience

One year of experience working with individuals with significant disabilities.

Under direct supervision, reviews and interprets education, vocation, psychological and medical information.

Four years of directly related experience.

Some positions may require specialized certification.

Teaches interpersonal skills to help inmates obtain and retain employment.

Knowledge of the Department of Corrections policies and procedures.

Knowledge of behavior modification techniques and philosophies used in corrections.

Knowledge of techniques used in crisis intervention.

Maintains records to go into permanent files.

Writes monthly reports to document work with inmates, achievements in progress, activities held or problems encountered.

Communicates with other counseling personnel to gain knowledge or understanding of counseling responsibilities.

Basic computer knowledge as it relates to web-based databases, word processing, email systems, and data entry
