Highly organized with demonstrated experience in task tracking and project management

Provides patient self-management support with a focus on empowering the patient/family to build capacity for self- care.

Support psychotropic medication management as prescribed by medical provider, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.

Provide patient education about mental health and substance abuse disorders and available treatments.

Participate in required continuing education opportunities.
