Behavioral Health Care Coodinator Jobs in Texas

Minimum one (1) year of experience in utilization or case management, discharge planning and quality improvement in a managed care setting.

Working knowledge of all federal, state, local and regulatory requirements.

Minimum three (3) years of nursing or social worker experience in the behavioral health field.

B.S. in Nursing OR four (4) years experience in a directly related field.

High School Diploma or GED (General Education Development).

Document Review Activities to include: Medical necessity for admission/procedure

Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.

Provide patient education about common mental health and substance abuse disorders and available treatment options.

Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.

Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.

Provider brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.

Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.

At least 2 years previous experience in social services or healthcare required

At least 4 years related experience preferred

Four-year Bachelor's degree or equivalent experience required

Work directly with patients and families as directed by licensed a BH clinician.

Work effectively with PCPs and office staff to assure satisfaction with Children's BH services.

Facilitate parent/child referrals to address social service needs and supportive therapy.

Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients

Administering benefits and reviewing treatment plans

Coordinating benefits and transitions between various areas of care if indicated

2+ years of experience in a related mental health environment including outpatient services

Dual diagnosis experience with mental health and substance abuse

Experience working in Intensive Outpatient setting
