JOB SUMMARY

The Case Manager position is responsible for care coordination through provision of a range of case management and support services to LYNC youth and families, in coordination with other LYNC staff, agency, and community providers. Works closely with LYNC Service Specialists in identifying and connecting youth and families to community resources.

KEY RESPONSIBILITIES

  • Develops a Risk Reduction Sustainability Plan (RRSP);
  • Acts as point of contact for youth and families during service delivery.
  • Engages in strength-based, culturally competent, trauma-informed and gender responsive interactions with youth and families.
  • Promotes a safe, welcoming environment with a focus on risk reduction and positive behavior change.
  • Participates and collaborates in all phases of LYNC: Engagement, Service, and Sustainability, as part of the LYNC team to provide services to referred youth and families.
  • Works as a team member to respond to client and programmatic needs.
  • Discharge planning including plans for follow up treatment and connection to community services and programs;
  • Attends all trainings and meetings as required for the position.
  • Contributes to agency visibility and development of community linkages by representing the Village in community groups or committees, as assigned by supervisor.
  • Conducts clinical assessment and engagement;
  • Works with community organizations to connect youth and families to services.
  • Contributes to agency organizational life by participating in agency wide task forces, committees or problem solving teams as appropriate.
  • Accurate and timely completion of documentation and reports, and any other documentation and reports required by agency and program policies and funding sources.
  • Implements and monitors RRSP with youth and family, modifying as appropriate during the course of service;
  • Structured case management, using Carey Guides;
  • Knowledge and implementation of program and agency policies and procedures.
  • Provision of direct services to youth and their families through care coordination which will include:
    • Conducts clinical assessment and engagement;
    • Develops a Risk Reduction Sustainability Plan (RRSP);
    • Implements and monitors RRSP with youth and family, modifying as appropriate during the course of service;
    • Structured case management, using Carey Guides;
    • Discharge planning including plans for follow up treatment and connection to community services and programs;
    • Works with community organizations to connect youth and families to services.
  • Partners with supervisor to identify areas and develop plans for professional growth.
  • Upholds all program expectations.
  • Accepts and performs all new assignments as determined by supervisor.
  • Assists in providing transportation to LYNC youth and families as needed.
  • Notifies supervisor immediately of any situation in which a youth may be at risk (i.e. physical abuse, sexual abuse, chronic neglect, un-safe home, etc.) and/or other sentinel event.
  • Develops and maintains collaborative relationships with other community agencies and organizations in consultation with supervisor, in order to provide comprehensive and effective treatment services for clients (i.e. schools, courts, community organizations, etc.)


Experience and Skills:

EDUCATION/ EXPERIENCE

  • Effective engagement skills.
  • Bachelors Degree in Criminal Justice, Sociology, or Human Services OR a minimum of three (3) years experience working with justice/system/agency involved youth.
  • May require weekend and evening hours.
  • Commitment to Results Based Accountability.
  • Excellent oral and written skills and ability to communicate comfortably with children, parents, and school personnel.
  • Strong community connections and knowledge base.
  • Demonstrated understanding of risk-needs-responsivity principles.
  • Must have current valid state issued Driver License.
  • Strong case management skills.
  • Experience working with youth involved in the juvenile justice system.
  • Cultural competent to serve targeted populations.
  • Proficient with word processing and spreadsheet applications. Possess solid general computer skills, including ability to work in a MS Office environment. Familiarity with specific applications used by the Village a plus.
  • Organized and able to work independently with minimal supervision.
  • Bicultural, Bilingual skills preferred.
  • Flexible and able to multi-task.
  • Knowledge of greater Hartford and New Britain area community resources.
  • Able to function safely in therapeutic situations.
  • Able to travel to various sites throughout greater Hartford and New Britain.
