Job Description
Overview


MHA in Ulster Country, Inc has a full-time (35 hours per week) Transitions Coordinator position available. The Transitions Coordinator coordinates and provides advocacy for young adults, between the ages of ages 18 and 26, are homeless or at risk of being homeless and have drug and alcohol / mental health issues.The goal of the program is to help transition-age youth function more independently and successfully across life areas to increase independence and promote recovery.Tasks include linking individuals to needed services, providing advocacy and support with community agencies, vocational exploration and skill building.


The schedule for this position is Monday - Friday 9am - 4:30pm.


The pay rate for this position is $20.06 per hour.


Responsibilities And Duties Include


  • Provide or coordinate consumer education and staff training on substance abuse issues and other needs as they arise.
  • Provide or coordinate services such as information, referral, emergency housing, clothing, transportation, legal and entitlement advocacy, peer support, vocational, life skills training and education, and crisis intervention when needed.
  • Identify and develop a safety plan to address any imminent safety concerns
  • Conduct a minimum of two face-to-face contacts per month with each individual to ensure continuous monitoring of progress and/or revising of Service Plan.


Qualifications And Skills


  • A clean, valid NYS driver’s license that will meet and maintain a satisfactory driving record consistent with agency and insurance standards and have been driving for at least 6 years.
  • Clearance by the NYS Office of Children and Family Services for instances of child abuse and/or neglect.
  • A Bachelor's Degree in a Human Services Field and 2 years’ relevant experience working with adolescents, and young adults who have MICA and mental health issues. OR An Associate’s Degree in a Human Services Field with at least 3 years’ experience, OR A high school diploma or equivalent and 5 years’ experience working in Human Services.
  • Ability to safely perform the essential functions of the position, with or without accommodations required by federal, state, or local law.
  • Clearance by the NYS Justice Center through fingerprinting for a Criminal History Records search.
