Transfer center nurses are a special breed. They are able to provide all the skill-based, compassionate care you’d expect from a nurse while also thinking outside the box. Our transfer center nurses engage team members to ensure timely and safe patient transfers. They help determine the appropriate level of care for transfer requests. They take nursing one step further and constantly strive to improve patient care

WHAT YOU WILL DO

• Obtain pertinent clinical information from the referring and admitting physician that is necessary to determine the appropriate level of care and bed assignment for the transfer request.
• Respond to all callers with the aid of an electronic database with service profiles, approved clinical protocols, and healthcare information facilitating of patient transfers.
• Work collaboratively with bed management and other team members ensuring appropriate patient admission order, placement and to implement plan of care.
• Collaborate to the appropriate facility based on patient preference, services required, geography, facility capacity/capability, and obtaining appropriate medical and administrative approvals.
• Facilitate, track, and document all hospital transfers.
• Communicate with physicians and their offices and outlying facilities if bed placement of a specific patient will be problematic.
• Follow specific protocols, policies, procedures, and utilize established resources demonstrating understanding of program requirements.
• Work collaboratively with team members to ensure exceptional consumer experience.
• Documents and monitors all activities associated with the transfer from initial physician referral to inpatient admission and patient placement.
• Assist in data collection, monitoring, evaluation, and analyzing of triage problems for the purpose of developing and implementing protocols, policies, and procedures.
• Relay information concerning the medical condition of the patient and the estimated time of arrival, to prepare physicians and facilities to receive patients.

WHAT IT TAKES TO SUCCEED

• Establish and maintain a high degree of awareness and knowledge of participating Franciscan Health hospitals, medical staff, services and special programs.
• Use problem-solving skills, combined with established department processes to determine the best course of action which will be satisfactory to the patient, the physician, and the hospital.
• Excellent verbal and written communication skills.
• Knowledge of Centers for Disease Control and Prevention (CDC) Standard Precautions.
• Excellent organizational skills and ability to multi-task.
• Emergency Response and Preparedness.
