Job Description
The Trans Peer Support Specialist provides prevention and harm reduction services to individuals at risk for infectious disease, overdose, and other health conditions associated with substance use and/or unstable housing at the Victory Connector, a harm reduction center for women and the transgender community. The Specialist conducts HIV/HCV and STI testing and care linkage services, community outreach, syringe exchange services, overdose education and naloxone distribution, and drop-in milieu oversight. The Specialist designs and implements health and support interventions for Trans and gender expansive clients. The Trans Peer Support Specialist reports to the Assistant Program Director.
Essential Functions:
  • Act in accordance with Victory Programs, Inc.’s Standards of Excellence & Philosophy Care at all times including maintaining professional ethics, boundaries and client confidentiality and working from a strength based perspective using evidence base practices (MI,CBT, Harm Reduction etc.)
  • Adhere to the Victory Programs Confidentiality Policies and Procedures at all times and in the course of all interactions.
  • Comply with agency protocols for data collection and record management.
  • Conduct HIV, Hepatitis C, and STI screening and linkage to care in the drop-in and mobile settings.
  • Attend weekly supervision, monthly staff meetings, and periodic professional development trainings and workshops as requested by supervisor.
  • Design and implement individual and group-level peer support interventions for transgender and gender non-conforming clients.
  • Coordinate education sessions/workshops on gender affirming procedures, hormone replacement therapy and risk reduction strategies to relevant people and communities.
  • Provide supported referrals, navigation, and appointment accompaniment for clients seeking health or social service care linkage.
  • Engage in Nonviolent Crisis Intervention and conflict de-escalation as needed in milieu space
  • Collaborate with team to organize daily activities, operations, and harm reduction programming.
  • Collaborate with Street Outreach staff to provide harm reduction services and navigation to individuals at risk for infectious disease, overdose, and conditions associated with substance use in the Newmarket area.
  • Work collaboratively with entire Mobile Prevention Team staff to coordinate trainings, psychoeducational and peer support groups in the drop-in center.
  • Provide syringe services, naloxone, and harm reduction supplies for individuals at risk for overdose of infectious disease transmission.
  • Facilitate verbal and written harm reduction counseling related to safer injection practice, wound care, overdose prevention, sexual health, vein and abscess care, and prevention of infectious disease transmission to clients.
Non- Essential Functions:
  • Participate and support the agency in program events, State House rallies, fundraising and advocacy events, rallies, walks and any additional functions.
  • Perform any other duty or task that may be requested from time to time by your supervisor or their designee.
Required Qualifications:
  • Strong computer skills, including experience with Microsoft Outlook, Excel and the internet.
  • Current TB assessment and current CPR certification (agency provided upon hire).
  • Ability to work in an environment that involves potentially hazardous materials (blood borne pathogens, discarded syringes, etc.) and situations that require safety precautions and/or the use of protective equipment.
  • Phlebotomy and specimen collection training/certification or a willingness to be trained.
  • A successful candidate uses their lived experience within the transgender and/or gender expansive community to support and empower clients on issues related to their health, wellness, and prevention.
  • Ability to work effectively with individuals of diverse race/ethnicity, sexual orientation, gender identity, sex and drug cultures, age groups, religion, country of origin, disability and economic status.
  • Excellent verbal communication skills including an ability to have open and affirming conversations about sex and substance use risk.
  • High school diploma or equivalent.
  • Working knowledge of HIV/AIDS and viral hepatitis, homelessness, substance use, STIs, and sex work as well as health-related social issues faced by persons who use drugs.
Preferred Qualifications:
  • Bachelor’s degree.
  • Bilingual capacity.
  • Previous experience in nonviolent crisis intervention, de-escalation, and milieu management.
  • Two years experience working in human services or harm reduction/prevention services.
