WHAT YOU CAN EXPECT

• Follow policies and procedures and maintain competency for equipment.
• Work and communicate directly with clinical managers, field staff, and physicians to provide appropriate intervention to decrease avoidable hospitalizations.
• Use trended data to facilitate making visits on a day when an in-person assessment is indicated.
• Fax trended data to physicians at specified intervals.
• Monitor daily transmissions from telehealth patients from assigned sites.
• Maintain patient confidentiality.
• Provide staff and client teaching as well as coordination of data with physician and nurse case manager.
• Maintain a caseload of up to 150 telehealth clients per one FTE.
• Troubleshoot and program monitors using remote programming technology.
• Coordinate with client/patient/case manager and plan of care.
