Part Time Position -16 hours per week

Schedule: 7:45am-3:45pm on Tuesdays & Wednesdays (7.5 hour days) and then either Monday or Thursday, 7:45am-12:45pm (5 hr day).

There will be some variability/flexibility with start and end times.

The schedule would be maintained for 36 weeks: early Sept through mid-Dec, and mid-Jan through early June.

  • Ability to work independently and as part of a team.
  • Basic computer skills, including data input and management.
  • Ability to work cooperatively with school staff, community mental health providers, and other community agencies on behalf of youth.
  • Experience assessing for suicidality.
  • Excellent oral and written communication skills. Comfortable presenting to groups of youth and adults.
  • Experience working with teenagers and parents/guardians.
  • Working knowledge of the DSM-IV.
  • Knowledge of current mental health best practices for youth and the application of evidence-based prevention and intervention strategies.

Provides social work assistance to patients and families as related to social determinants of health.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Assists patients/families with obtaining community assistance by referral to proper resources.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.
  • Develops and maintains working relationships with community health, welfare, and social agencies and seeks creative means to assist the patient with his/her needs.
  • Active participant with other members of the team regarding opportunities for improvement in standard work.
  • May consult with members of the multi-disciplinary team in liaison with community resources to support the patient's treatment.
  • Interviews patients, assesses priorities and documents case activity.

  • Master's degree in Social Work

PHYSICAL REQUIREMENTS

  • Occasional driving.
  • Occasional lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Rare repetitive foot/leg and hand/arm movements and squatting.
  • Occasional use of vision to identify and distinguish colors.
  • Occasional bending, stooping, climbing, twisting and gripping.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more).
  • Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
  • Frequent keyboard use/data entry.
  • Frequent reaching, sitting, standing and walking.
