Social Worker - Msw Hospice- Antigo

We are seeking a SOCIAL WORKER - MSW in Antigo, WI at Aspirus Langlade Hospital – Hospice who supervises established policies and procedures, assists patients and families in coping with problems resulting from serious illness and declining medical status. The Social Worker provides centered, family focused services using professional expertise and skills. As a core member of the hospice/palliative medicine interdisciplinary team, assists patients/families in clarification of goals and plan of care. The Social Worker works with patients/families to obtain psychosocial history, assists with identified psycho-social & environmental needs, directs patients/families to designated community agencies or resources as necessary, and provides case management services according to the interdisciplinary plan of care. The Social Worker facilitates formal and informal meetings regarding patients and family communications and decision making, care coordination, advance care planning, bioethical considerations, patient advocacy, adjustment to illness and related issues, and discharge planning. The Social Worker will coordinate team efforts with appropriate hospital support departments and community providers and organizations to ensure continuity of care. The Social Worker will provide guidance and counseling to patients/families who are affected by incurable illness and/or approaching end of life. The Social Worker provides these necessary social work services to LeRoyer Hospice and Palliative Medicine patients/families under the direction of a physician. The Social Worker reports directly to the Director of Hospital and Palliative Medicine.

PROFESSIONAL EXPERIENCE/QUALIFICATIONS

• One year of experience in a health-related field is required, with two years of experience preferred. Previous hospice and palliative or clinical experience in counseling and casework needs of terminally ill is beneficial.
• Master of Social Work from a Council of Social Work Education, accredited school of social work; current state of licensure. Valid driver’s license. Current auto insurance.

Hours: .8 full-time, 64 hours every 2 weeks. Day shifts from 8am – 4:30pm

Aspirus Langlade Hospital is a 25-bed critical access hospital located in Antigo, Wisconsin. The hospital admits 1,300 inpatients annually with 62,000 outpatient visits each year. The Emergency Department has more than 12,000 patient visits each year. We average 180-200 births per year and perform 1,500 surgeries annually. The hospital holds The Joint Commission-National Quality Gold Seal Approval.
