Social Worker Jobs

Responsible for arranging all new admissions to the facility. Once admitted, this position will take the lead on coordinating care for short-term rehab residents. Coordinating care to include facilitating care conferences with families and/or residents, attending weekly Medicare meetings, conducting appropriate MDS assessments, developing care plans, and setting up discharge services as needed.
