• $30.00/hour and up DOE**

We are always looking for talented professionals to join our growing team
On an average day as a Social Worker, you will:

• Y ou will have the opportunity to collaborate with The Director of Clinical Services and other clinical team members (physicians, chaplains, therapists) to contribute to that mission. These passionate team members will depend on you to p rovide social services to patients and their families and to be a reliable go-to problem solver.
• Identify and utilize appropriate community resources and assessing patient/family ability to access them
• And perform additional, related duties as needed
• Offer short-term individual counseling, crisis intervention
• Identify support systems available to reduce stress and facilitate coping with end-of-life care
• Assist with funeral planning issues and transfer of responsibilities regarding fiscal, legal and health care decisions
• Assist in providing information and preparation of advance directives

Desired Skills/Qualifications

• Must have a active license in the state you are applying
• A Masters degree in social work from a school accredited by the Council on Social Work Education is strongly preferred
• A passion for providing care to patients and their families during and after the dying process
• One year of hospice related experience preferred
• Must have a valid drivers license, auto insurance, and reliable transportation for the purpose of traveling to our patients
• A desire to work in a team environment
• Excitement for doing work that makes a difference
