Job Description
Position Summary:
Full-time and Part-time Social Worker (MSW): Seeking both a full-time and a part-time social worker to provide psychosocial support for seriously ill patients and their families. Responsibilities include performing psychosocial assessments, education and completion of advance care planning, supportive counseling, and assistance with obtaining needed resources, including placement. You will work alongside your clinical and operational team members to ensure provision of high-quality hospice services.
Knowledge, Skills & Abilities:
  • Excellent interpersonal skills
  • Ability to plan, prioritize and coordinate
  • Ability to integrate core values, integrity, and accountability throughout all clinical practices.
  • Demonstrate the ability to function in a variety of care environments, including home settings, skilled nursing centers, assisted living communities, etc.
  • Ability to value and consider the perspectives and backgrounds of all parties.
  • Proficient in Microsoft Office (excel, word, outlook, power point) & Zoom / TEAMS
  • Knowledge of the Big 5 clinical expenses (labor, supplies, pharmacy, DME and mileage)
  • Knowledge of federal, state, and local regulations r/t Hospice
  • Ability to contribute to initiatives and processes within the organization.
  • Strong organizational skills and ability prioritize tasks, detail oriented and flexible
  • Knowledge of Philosophy of comfort, dignity, autonomy, quality of life, and empowerment
  • Practices safety and infection control methods
  • Ability to understand and apply information with which to contribute to the organization's strategic plan.
  • Knowledge of Medicare Conditions of Participation for Hospice
  • Knowledge of quality standards and reporting practices for hospice and palliative care
  • Ability to work under pressure to meet deadlines
  • Convey warmth, confidence, and competence
  • Excellent problem-solving skills
  • Knowledge of common grief responses
  • Ability to challenge processes that are inefficient/ineffective while contributing to the solution
  • Practices within the scope of license, certification or registration, where applicable, and as required by law and regulation
  • Ability to manage interactions to provide service and to support the organization.
  • Ability to effectively work virtually when needed
  • Knowledge of Adult learning & training methodologies
  • Excellent oral/written skills
Minimum Qualifications:
Education
  • At least one (1) year experience as a Social Worker in a healthcare and/or hospice setting is required; three to five years preferred.
  • BSW - Considered
  • Master of Social Work degree– Preferred
