Job Description


      Position Summary:
      This position provides patient centered direct social work services to patients and families within the hospital setting and serve as an integral member of the interdisciplinary healthcare team. The Social Work Care Manager will work to identify potential social work needs/supports in collaboration with the inpatient and ambulatory care teams, the patient and the patient’s external support system (if applicable). The Social Work Care Manager will consistently and thoughtfully apply social work values and continuous quality improvement in daily work.



      Education/Experience:

      Bachelor’s Degree in Social Work required from a program accredited by the Council on Social Work Education (CSWE), the Canadian Association of Schools of Social Work (CASSW), or a similar accreditation body designated by the Minnesota Board of Social Work. Previous healthcare or social work experience preferred.



      Special Knowledge/Skills/Certifications/Licenses:

      • Sensitivity to cross-cultural issues
      • Ability to establish and maintain effective working relationships with patients, families, colleagues, and community organizations
      • Knowledge of the values, principles and methodologies of social work
      • Ability to deal with change and ambiguity
      • Ability to organize and prioritize multiple assignments within workload
      • Ability to integrate social and medical data to make psychosocial assessments and implement social work treatment plans
      • Support and educate other disciplines when appropriate
      • Ability to work effectively as part of an interdisciplinary team

      Licensure: Licensed Social Worker (LSW), MN Board of Social Work

      Additional Information:
      The BSW/MSW Hospital Social Worker coordinates discharge planning for patients at Methodist Hospital. Hours are 8-430p with a weekend rotation (every 6th).

      Bachelor's or Master's of Social Work with license required. Experience in hospital or healthcare setting strongly preferred.
