About Your Role

• Conduct regular visits and delivers recovery-oriented education to the individual receiving services and their family members.
• Fostering the recovery process, aiming to inspire hope, facilitate healing, empower individuals, and foster meaningful connections.
• Providing support to individuals in selecting the ideal community living option that meets their needs and preferences.
• Facilitating collaborative planning of personalized service plans with the individual receiving services and service providers arranging meetings for plan adjustments when necessary, and leading these gatherings as the chairperson.

Requirements

• Current and valid Massachusetts Class D Motor Vehicle Operator’s license. (Based on the assignment)
• Knowledge of laws, rules, regulations, policies, procedures, standards, and guidelines governing assigned agency and unit activities.
• A Bachelor’s degree in a related field may substitute for two (2) years of the required experience
• Knowledge of agency forms and documents
