As an IHC Social Care Manager, you will serve our patients as an advocate, coach, and connector - working closely with patients, caregivers and other members of the Intensive Home Care team to identify and eliminate socioeconomic and psychosocial barriers to health and self-efficacy. This will require you to develop individualized care plans that align community, clinical, and financial resources; provide coaching, improve coping skills, self-management, and caregiver assistance; re-enforce disease-specific education; and facilitate advanced care planning discussions, including palliative or hospice care transitions when appropriate. You will examine, challenge, and seek to expand how the community environment supports the members’ needs, and work closely with IHC’s multidisciplinary team.
Responsibilities will include:
  • Provide social care screenings, resource alignment, & coordination services to maximize the effectiveness of the Intensive Home Care program:
    • Conduct social needs screenings and create personalized action plans tailored to members’ needs
    • Provide emotional and caregiver support
    • Assist in navigating complex family/support issues
    • Facilitating conversations about end-of-life planning
    • Consult with members, caregivers, and qualified healthcare professionals on long term care needs and facilitating achievement of those requirements
    • Place/follow through on behavioral health referrals
    • Investigate concerns for patient abuse or neglect

  • Work with family members/caregivers to help them in understanding the member’s needs and how they can be supportive
  • Build relationships and coordinate with key local market and internal stakeholders

  • Assist in navigating complex family/support issues
  • Establish trust and build strong relationships with our patients
  • Know how to balance multiple competing priorities from complex patients
  • Investigate concerns for patient abuse or neglect
  • Conduct social needs screenings and create personalized action plans tailored to members’ needs
  • Provide emotional and caregiver support
  • Interdisciplinary collaboration with Intensive Home Care team (including clinicians, case managers, community health workers, and outreach specialists) to ensure cohesive, patient-centered care for high-needs patients

Desired skills and experience:

  • Health insurance experience preferred (Medicare Advantage a plus)
  • Master’s degree in social work and active applicable state license
  • Bilingual in Spanish preferred
  • Have a minimum of 3 years working in medical social work, case management, or care coordination

Salary Range: $70,000 - $74,740
