Job Summary:

Leads, directs, and executes a network of skilled nursing facilities to meet the needs of SSM patients across the continuum of care.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Responsible for the regional operation’s ongoing compliance with local, state, federal, licensure and accreditation guidelines and regulations. Must comply with the corporate compliance program and immediately report any operational divergence from standards.
  • Utilize project management, consultation, facilitation and execution skills for a range of strategic initiatives that will require planning and analysis for a broad array of complex clinical and logistical issues impacting SSM Health clinicians as they provide care in non SSM Health sites and assist them in developing and implementing systems of care (care pathways, EMRs) to manage SSM Health patients across the continuum of services.
  • Develops growth strategies and business plans in cooperation with practice or department leadership.
  • Supports financial, budget and business goals. Ensures that staffing is in place to meet the demands of the providers' work schedules.
  • Makes effective use of the resources for new program development, systems improvement, best practice development and promotion.
  • Provides up-line management with reports as requested. Leads the development, implementation and management of all SNF service provider relationships and processes. Strategically manages network of service providers. Tracks key financial and production indicators.
  • Oversees daily operations of practice or department including but not limited to services, site policies, staff development, and patient flow.
  • Performs other duties as assigned.
  • Develops collaborative relationships and contracts with skilled nursing facilities, long term care, inpatient rehab services that are not part of the SSM Health system.
  • Understands the importance of communication with all levels of the organization, community, and customers. Create and implement a communication plan which meets the needs of staff, senior management, customers, and the community within the program’s service area.
  • Monitors budgets, regulatory compliance, contracts and vendor relations. Assesses quality of and coordinates patient care services with patients, staff, physicians, and other providers.
  • Works in partnership with the SSM Health Regional Management Teams to establish care processes to manage patients in the SNF provider network across the continuum of care. Promotes and maintains an atmosphere of customer service throughout all levels of the program. Provides opportunities for staff to receive continuing education about customer service and interacting with difficult people. Follow up and resolve customer complaints as needed.
  • Manages the daily operations of the assigned practice site(s) or department(s). Serves as a liaison between physicians and employees, and between the practice or department and other clinical and administrative areas.
  • Develop action plans to drive performance improvement. Implement benchmarks and performance criteria for the Skilled Nursing Facilities program. Provide senior management with accurate performance information. Develops the strategic direction and budget to support the SNF network of providers.
  • Provides a personal example of living the principles of continuous quality improvement whereby decision-making is driven by data, and customer-supplier relationships are facilitated both within and outside the System.

EDUCATION

  • Bachelor's degree

EXPERIENCE

  • Ten years' experience with five years in management

PHYSICAL REQUIREMENTS

  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
  • Occasional bending, stooping, kneeling, squatting, twisting and gripping.
  • Frequent keyboard use/data entry.
  • Rare climbing.
  • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
