Job Description
A health services organization in New York City is currently seeking a hardworking and dedicated professional to join their staff in Queens as a Service Coordinator. In this role, theService Coordinator will be responsible for assisting the applicants and participants in obtaining and coordinating the services that are necessary to remain in or return to their home.


About The Opportunity


  • Caseload: 10 to 15
  • Hours: 8am to 4pm or 9am to 5pm
  • ????Schedule: Monday to Friday (1 to 2 days in the field; 3 days office)
  • Patient Population: Elderly and/or disabled
Responsibilities:


TheService Coordinatorwill


  • Perform other duties, as needed
  • Maintain case records consistent with all contacts and necessary documentation
  • Maintain consistent and open communication with the RRDS and waiver participants
  • Coordinateand schedule waiver and non-waiver services
  • Act as the participant's advocate, provide unbiased assistance to the participant with the selection of providers, and assist the participant in locating a place to live in the community
  • Conduct face-to-face meetings with the waiver participant at least monthly to monitor health and welfare needs
  • Complete Individualized Service Plans and Plans of Protective Oversight (PP0) andmeetwith participants in order to gain an in-depth understanding of their goals
  • Maintain a tracking system that monitors all areas of compliance
  • Assure the timely implementation of the approved service plan
Qualifications:


  • Solid assessment and documentation skills
  • Bachelor's and/or Master's Degree in Psychology, Social Work, and/or a related field
  • Experience working with other service providers and Medicaid benefits
  • Excellent communication skills (written and verbal)
  • Knowledge of waiver services, as well as current knowledge of informal supports within the participants' communities
  • Highly organized
  • Strong attention to detail
  • Great interpersonal skills
  • 2+ years of experience providing service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors
