Babies Can’t Wait (BCW) is Georgia’s early intervention program that offers a variety of coordinated services for infants and toddlers with special needs, from birth to three years-of-age and their families. The purpose of BCW is to:

  • Provide early identification and screening of children with developmental delays and chronic health conditions.
  • Support family members and caregivers to enhance children's learning and development through everyday learning opportunities.
  • Improve the developmental potential of infants and toddlers birth to age three, with developmental or chronic health conditions.

Purpose

Provides the local health district with Service Coordination (case management) services, support and education to children and families enrolled in the BCW program by using the Primary Service Provider (PSP) model.


Job Responsibilities

The Service Coordinator position provides case management services to children aged birth to 3 years at-risk for/with developmental delays and/or an established medical condition. This includes initial screenings, intake interviews, coordination of referrals, plan development, completion of coordination notes and ongoing contacts with the family. The specific required tasks of each caseload vary based on the business needs of the district office as well as the needs of the families. Some Service Coordinator positions serve as Intake Coordinators and will have the primary responsibility of conducting intake meetings and evaluations. Ongoing Service Coordinators will support children with active Individualized Family Service Plans (IFSP). Both roles, however, will be expected to complete all duties as a Service Coordinator when necessary.


Duties include but are not limited to:

  • Know, understand and follow guidelines for implementing the Primary Service Provider (PSP) Model.
  • Facilitate, coordinate, implement and monitor services for children and update IFSP as necessary.
  • Conduct and complete intake with family.
  • Work with Multidisciplinary team of professionals.
  • Assure close working relationship with other human services agencies as they relate to assigned caseload.
  • Make necessary referrals and coordinate services as recommended by PSP team.
  • Oversee the development and implementation of the Individualized Family Service Plan (IFSP).
  • Ensure that transition planning, case transfer, and case closure occur consistent with state and federal requirements.
  • Know, understand, and follow the Health Insurance Portability and Accountability Act (HIPAA) and Family Educational Rights to Privacy Act (FERPA).
  • Maintain records, correspondence, service plans and authorizations for each provider service.
  • Attend team meetings and other scheduled meetings.
  • Coordinate evaluation of child’s development to determine program eligibility and need for services.
  • Assist in the development, maintenance, and processing of the child’s early intervention case record and ensure deadlines are met.
  • Other duties as assigned by supervisor.
  • Provide families with information about the early intervention service program.

Minimum Qualifications

Candidate must have a Bachelor’s degree from an accredited college or university which includes 12 college credits in early intervention, early childhood special education, early childhood education, child development, special education, elementary education, family counseling, family studies, social welfare, psychology or other comparable social sciences, and one year of full-time or full-time equivalent experience working with or providing counseling to children, families or individuals with disabilities


OR


Bachelor’s degree in human services field from an appropriately accredited institution and three years of professional experience working with the population served


OR


An equivalent combination of education and experience.

Case management experience, excellent communication skills, strong interpersonal skills and familiarity with community resources are also strongly recommended.

  • Computer literate and ability to learn new skills related to current technology.
  • Must be passionate and committed to early intervention.
  • Must pass a background check and complete fingerprinting.
  • Must be professional, self-motivated, energetic and genuine.
  • Copy of official transcripts.
  • Must have reliable transportation to travel to scheduled appointments.
  • Certificate of Automobile Insurance.
  • Must enroll in fee-for-service Medicaid.

** This position is a contract position. Caseload size and individual schedules are flexible and determined based on district need and provider availability. Reimbursement for services provided are based on current Medicaid Rates.


Master's degree in a related field from an accredited college or university AND eligibility to be licensed OR Bachelor's degree in a related field from an accredited college or university AND Two years of experience in social service delivery.
