Position Description


The Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) assumes responsibility and accountability for the application of the nursing process and the delivery of patient/client care and/or contracted services under the supervision of a registered nurse or licensed designee. The LVN/LPN provides service in accordance with Maxim philosophy, standards and policies


Essential Duties And Responsibilities


  • As appropriate, provides input and suggestions regarding areas in which policies, procedures and workflows.
  • Evaluates patient/client and provides a safe, comfortable and therapeutic patient/client environment
  • Demonstrates on each assignment the provision of skilled nursing care rendered in accordance with prescribed orders
  • Communicates observations and significant changes in patient/client status to the Director of Clinical Services and/or Clinical Supervisor /Case Manager or licensed designee
  • Willingness to participate in skills advancement training or novice nurse program if applicable
  • Notifies Director/Supervisor if applicable on the following;
  • Standards of Nursing Practice
  • Patient Bill of Rights
  • Supports professional practice by observing the following;
  • Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or contracted services
  • Monitors the safety and effectiveness of the environment and equipment including universal precautions
  • Current CPR as applicable, for direct patient care
  • Current Health Certificate (per facility contract or state regulation)
  • Must meet all federal, state, and local requirements Compliance and Ethics Expectations:
  • Physician notification to obtain necessary orders for intervention(s) per state regulations b. Changes in patient/client condition
  • Participates and successfully completes the company’s compliance program requirements and adheres to the Code of Conduct, Company policies, and applicable federal and state requirements
  • Participates in activities to facilitate good interpersonal communications
  • Sets an example for other employees regarding how the Company’s Code of Conduct and Compliance Program is applied and observed every day when dealing with customers, business operations, or other team members
  • State and Local Laws
  • Adheres to Maxim's policies and procedures and applicable protocols
  • Strong verbal and written communication skills
  • Attends case conferences, team meetings, education in-services as required
  • PPD or chest x-ray if applicable
  • Nurse Practice Act
  • Maxim Healthcare, Nursing, and Client’s Hospital Policies
  • Participation and completion of Maxim’s Competency Program
  • At least one year prior professional nursing experience, preferred
  • Assures patient safety at all times and recipient of services provided
  • Reports potential violations of company policy, Code of Conduct, and/or applicable laws and regulations to the company hotline, through the chain of command, to the Compliance and Ethics Department, or through other channels made available by the company for reporting potential violations
  • Promotes an environment in which other employees are encouraged to report potential violations
  • Standards of Accrediting Body (ACHC, Joint Commission, etc.)
  • Documents all physician collaboration/coordination of care in the medical record and communicates this information as needed
  • Identifies and documents changes in patient’s both physiologic and psychosocial condition
  • Graduate of an accredited practical nurse or vocational nursing program, preferred 2. Licensed Practical Nurse or Licensed Vocational Nurse in specific state
  • Verifies that documentation reflects patient/client collaboration, participation in plan of care, expected patient/client outcomes, and selected nursing interventions
  • Identifies and prioritizes actions based upon patient/client care requirements
  • Successful completion of new hire training as applicable to job site
  • Complies with accepted professional standards and practice
  • Standards of Patient Care
  • Performs interventions, treatments, and medication administration, according to identified priorities, plan of care, facility policies and patient/client care outcome standards as applicable. Monitors and documents effectiveness of interventions, treatments, and medications
