Job Description
Summary:
Responsible for coordinating clinical care and transitioning patients receiving complex infusible drug therapies from the acute care environment to be continued at home. This position works directly with patients, licensed independent providers, case management, pharmacist and home health services to coordinate care for patients in need of infusion services post discharge.
Detailed responsibilities:
  • Works in collaboration with home infusion intake to verify eligibility and clinical appropriateness for the prescribed home infusion service.
  • Performs patient and family education and training on home infusion services including administration drug therapy, infusion delivery methods, aseptic techniques and IV access and intravenous line access maintenance.
  • Maintains communication with all stakeholders, including patient and family regarding all aspects of therapy, including potential issues that may contribute to therapy delays, denials or prior authorizations related to service benefits. Maintains exceptional on-going communication and documentation regarding referral status and final disposition of infusion cases.
  • Participates in quality program initiatives and improvement activities including incident reporting and or adverse drug events reports
  • Serves as main liaison between the pharmacy, referral resources, patient and family members until discharge plans are finalized.
  • Performs order entry clarification, clinical progress note, plan of care documentation for physicians and other home care providers to ensure appropriate orders are available for patient discharge.
  • Responsible for care coordination for home health services related to infusion therapy prescribed if services cannot be met by MHS home health partners.
  • Gathers the appropriate medical information necessary to support need for infusion service . Utilizes nursing clinical skills in a collaborative process to identify, coordinate and facilitate exceptional healthcare outcomes for patients.
  • Performs individualized clinical screening assessments of patient's disease and prescribed therapy to ensure safe transition of care from hospital to the outpatient environment.
  • Performs on going documentation regarding referral status and final disposition.
  • Performs bedside infusion therapy site evaluation, transfer of infusion equipment for high risk continuous intravenous infusion start up care transition prior to hospital discharge (i.e. inotrope and parenteral nutrition therapies).
  • Responsible for reviewing 100% of all infusion referral service orders for accuracy and appropriateness. Triages referral to the appropriate service provider.
