Job Description
Unit Managers are primarily responsible for overseeing day to day resident care, supervising, directing, and developing nurse staff, and reporting to the Director of Nursing to ensure quality patient care within a long-term care center or skilled nursing facility. Unit Managers are required to be dependable and personable liaisons between residents and their families. Unit Managers are tasked with ensuring their unit complies with federal and state regulatory standards and follows company policies and procedures. The daily tasks of a Unit Manager generally include:


  • Collaborates with the Director of Nursing, Nurse Practitioners and Physicians to plan and maintain resident care
  • Acting as a main point of contact between residents and their families
  • Completes admission and discharge assessments
  • Assists in developing, implementing, and adjusting patient care plans as needed
  • Supervises, counsels, and provides discipline when necessary, to nurse staff to ensure the highest quality care possible
  • Audits and completes charting, evaluations, medical forms, and other administrative tasks
  • Addressing and preventing concerns from family members
  • Identifies procedural problems and works with the Director of Nursing to resolve issues
  • Delivers hands-on care and medicine distribution as needed
  • Ensures their unit meets federal and state regulatory standards, as well as company policies and procedures
  • Oversees and audits changes made to care plans
  • Maintaining and restoring health of the resident
  • Evaluates, prioritizes, and meets the care needs of each resident


JOB QUALIFICATIONS:


  • At least one year of experience in supervisory capacity in a hospital or extended care facility, including experience in geriatric nursing.
  • Ability to manage and supervise a nursing unit. must show interest and ability when teaching others and assisting in their skills.
  • Ability to understand resident charts, doctor’s orders, nursing care plan and medication orders, and to communicate medical information to residents and staff.
  • Capacity to grasp new practices and standards and apply their use where appropriate.
  • Good mental and physical health, sound judgment and high moral standards, and a sincere desire to work with those with limited capacity for self-care.
  • Graduate of an accredited school of nursing. Current registration with the Virginia State Board of Nursing.
  • Ability to make independent decisions.


SPECIFIC DUTIES:


  • Validates those medications are given as ordered or responsibly withheld, including appropriate documentation and notifications.
  • Responsible for knowing Fire, Safety and Disaster Manuals, including direction of subordinates in prevention drills, emergencies, and other related situations.
  • Responsible for keeping DON and Supervisors fully informed regarding needs, unusual occurrences, problems, and accomplishments to establish/maintain lines of communication and to enhance continuity of quality resident care.
  • Assures Wound and Pressure Sore development is investigated per policy, assessed weekly (including measurements), and orders treatments and interventions are evaluated and amended, as necessary. Provides written report to DON for QAA.
  • Supervises and evaluates the performance of staff nurses, CNAs and Unit Secretaries who work on unit.
  • Plans, implements, and coordinates all nursing for unit, including nursing services for SNF resident on unit.
  • Responsible for promoting and maintaining good interpersonal/interdepartmental relationships, including carrying out administrative procedures in conformity with policy.
  • Validates those medications and treatments that are ordered have appropriate diagnoses and reasons for giving.
  • Responsible for in-depth assessment of new admissions and re-admission, including completion of at-risk Assessments (Falls, Wt. Loss, Pressure, etc.). Assures all required documentation (MD orders, History & Physical, Transfer Sheet, Chest X-ray, etc.) arrive with admissions. Responsible for obtaining required documentation from resident/family regarding Preferred Intensity of Care Advanced Directives, No CPR, and use of restraints.
  • Assures appropriate MD notification per policy and within professional standards, including physician’s orders obtained for nursing procedures requiring such an order.
  • Maintains current resident “problem list” of events, occurrences, symptoms, orders, etc. that require follow-up; and validates appropriate documentation and interventions.
  • Any other duties as requested by DON, ADON or Administration.
  • Responsible for on-going knowledge of and implementation of resident care-plans goals and interventions.
  • Assists DON in assuring quality nursing care and agency compliance with Medicaid, Medicare, and State Health requirements.
  • Assists In-service Director in coordination of orientation of new trainees and Inservice on nursing department employees on unit.
  • Responsible for coordination, evaluation, and documentation of ongoing Restraint Reduction Program and for tracking of side rail usage for QAA Committee.
  • Coordinates through DON, Supervisors and Staff Nurses all unit management and resident care tasks.
  • Responsible for enhancing professional development of staff in areas of creativity, skills behavior, and personal appearance.
  • Assures required monthly summaries are completed with require supporting documentation in compliance with Medicaid, Medicare, and State Health documentation requirements.
  • Assures narcotics and controlled drugs are in correct count for unit.
  • Responsible for care/utilization of supplies/equipment on unit.
  • Assures resident behaviors are documented with appropriate and least restrictive interventions attempted before chemical/physical methods of modification.
  • Assists the DON and MDS Coordinator in review of resident’ charts, including quality indicators to ensure medical and nursing compliance with Medicaid, Medicare and State health requirements.
  • Plans effective administration of all nursing services on unit.
  • Assists MDS Coordinator with assessment of residents’ problems and care plan initiation and implementation, including periodic resident reassessment, care plan evaluation and revision within regulatory-set time frame.
  • Assures physician’s orders are properly transcribed and implemented with appropriate supporting nursing documentation.
  • Assures documentation and follow-up of all incidents on unit per policy in timely manner. Investigates all falls, skin tears and bruising, and provide written report to DON for QAA. Assures appropriate interventions for injury prevention in place and in use.
  • When a resident is transferred or discharged, assures complete resident transfer instructions sent to the other facility and written discharge instructions given to the resident and/or family.
