Job Description
Title: RN Short-Term Rehab. Nurse Manager


Supervisor: Guide/Director of Nursing


General Description


The Assistant Guide/Clinical Nurse Leader performs nursing duties within the scope of practice as defined by the State of Connecticut and provides nursing care to household Residents in accordance with the Core Values of Jewish Senior Services and practices within the scope of the role of RN/LPN. The Assistant Guide/Clinical Nurse Leader assumes the responsibility for the overall delivery of care and supports the household team to ensure quality of care and quality of life.


Job Duties / Responsibilities / Expectations


  • Communicates and collaborates effectively with Resident/patients, families, staff and other household members of the interdisciplinary team to meet Resident/patient and unit needs.
  • Annually completes the Core Competencies for the role of an RN.
  • Provide assessments when changes in Resident's/patient's condition occur. Verify and co-sign all orders transcribed by LPN's.
  • Contributes to the MDS and care plan by collecting data in an ongoing and systematic manner.
  • Documents accurate information in a timely and concise manner as required per regulations and Jewish Senior Services.
  • Provides nursing care appropriate to the needs and abilities of Resident's including: medication and treatment administration; mentoring; implementation of the interdisciplinary care plan, etc.
  • Facilitates resolutions with Residents and household team members in shared problem solving, decision making and planning.
  • Works weekends and holidays on a rotating basis according to the needs of household.
  • Clarifies, transcribes and carries out physician orders.
  • Participates in QAPI Program.
  • Participates in educational programs, inservices, meetings and workshops and utilizes current literature to enhance professional skills and knowledge.
  • Participates as part of a team, not always the leader.
  • Participates in problem solving complex medical/psychosocial situations related to Resident choice and risk
  • Follows policies and procedures of JSS.
  • Understands the importance of consistency of caregivers for Residents.
  • Supervise care provided by LPN's and CNA's to assure compliance with the interdisciplinary care plan.
  • Identifies, promotes, and models person-centered care.
  • Maintains confidentiality at all times.


Assessments


  • Records and appropriately reports the results of the nursing assessment to other members of health care team.
  • Participates in collection of data from Resident's or other appropriate sources.
  • Recognizes the relationship of assessment findings as related to Resident's diagnosis.
  • Verifies and interprets data collected.


Planning


  • Identifies Resident's needs.
  • Determines goals and interventions.
  • Formulates plan of care.
  • Determines all immediate needs and interventions.


Implementation


  • Implements plan of care including securing resources.
  • Assigns, delegates, and oversee licensed and unlicensed staff to ensure positive outcomes.


Evaluation


  • Evaluates effectiveness and achievement of expected outcomes.
  • Modifies plan of care as required.


Reports And Records


  • Demonstrates effective communication skills with Residents, representative of Resident, physicians, and all members of the household team.
  • Documents in accordance with organizational policies and procedures using the electronic medical record and all other communication tools.
  • Responsible for accurate and timely documentation.
  • Gives and receives a complete report at the beginning and end of assignment: includes correct counting and documentation of controlled substances.
  • Receives and transcribes physician orders according to policy.


Mentoring And Counseling


  • Mentoring/Counseling uses established policies and protocols.
  • Makes referrals to and for appropriate resources.
  • Identifies learning needs.
  • Participates in mentoring and counseling of Residents and families.


Collaborating


  • Participates in the household to organize work of the household and establish priorities.
  • Is accountable for quality nursing care.
  • Initiates, coordinates, plans, and provides nursing care within the multidisciplinary team.

  • Graduate of an approved school of nursing.
  • Must be able to lift 50 pounds or more.
  • Must be able to use equipment, push Residents in wheel, shower and Geri chairs.

  • Genuine interest in geriatric care and Residents of Jewish Senior Services.
  • Must be able to handle stressful situations.
  • Currently licensed as a RN in good standing with the State of Connecticut.

  • Previous long-term care experience preferred.
