The VNA Health Group is seeking a Registered Nurse Hospice Liaison to join our dedicated Hospice Team in Ocean County. The position would be responsible for assessing patients for hospice needs and transition of service and coordinating total care for hospice patients and their families.

Responsibilities

• Assists in developing and implementing a discharge program at Jersey City Medical Center
• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
• Assesses the needs of the patients and provides counseling to patients and their families and coordinates the transition to hospice to promote continuity of care
• Assists in managing and monitoring the quality and appropriateness of the services and programs
• Establishes and maintains liaison with physicians, external and internal customers
• Conducts community outreach activities to improve community access to hospice care
• Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
• Adheres to the organization’s policy in regards to absenteeism and appearance
• Adheres to agency infection control and safety policies

Qualifications

• Excellent clinical skills and experience in hospice nursing and community health nursing
• Strong computer skills with proficiency in MS Office Suite
• Excellent verbal and written communication skills
• Excellent customer service, organizational and interpersonal skills
• Bachelor’s degree in Nursing required

Working Conditions/Physical Demand : Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license
