Job Description
Job Summary


Under the supervision of the Manager Case Management, the RN Case Manager Lead functions as a resource to the Case Management Team. The RN Case Manager Lead assists the manager with the day-to-day operational functions of the case management department in addition to their patient caseload. This position will assist with teammates' adherence to organization and departmental policy and procedures, talent acquisition and orientation, and monitoring performance to quality and productivity standards to ensure operational excellence. The RN Case Manager Lead must follow established standards and practices in accordance with the South Carolina Nurse Practice Act (RN) in provision of case management services.


Minimum Qualifications


Education: Bachelor's degree in Nursing from an accredited school or college of nursing. Case Management certification preferred.


Experience: At least 1 year of full-time work experience as an RN Case Manager required Previous leadership experience preferred.


Licensure/Certification: Currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC. Case Management certification preferred.

Knowledge/Skills: Knowledge of organizational policies and procedures to provide patient care. Skill in preparing and maintaining effective working relationships. Demonstrated ability to communicate effectively at various organizational levels. Knowledge of common safety hazards and precautions to establish a safe work environment. Adheres to the Standards of Practice for Case Management. Demonstrates knowledge of case management concepts along the continuum of care. Ability to successfully articulate the process of attaining goals and outcomes of case management. Ability to apply clinical knowledge in a case management role. Demonstrate understanding and compliance with regulations and recommendations of outside regulatory bodies about providing case management services. Excellent verbal and written communications skills. Ability to engage and collaboration with the patients and significant others in the case management process. Knowledge of HIPAA and the ability to apply during the case management process. Demonstrates ability to prioritize activities daily and adapt effectively to changing priorities. Ability to case manage diverse populations without applying one's own personal values. Ability to work with minimal supervision within case management scope of practice based on appropriate licensure. Ability to think critically and analytically and work with minimal supervision.


Other: Ability to organize and prioritize multiple tasks quickly in a fast-paced environment. Must be self-directed and able to work independently. Skill in applying and modifying the principles, methods and techniques of professional nursing to provide on-going patient care. Skill in identifying problems and recommending solutions. Skill in establishing and maintaining effective working relationships with patients, hospital, medical staff and other stakeholders. Ability to analyze situations and apply critical thinking. Ability to communicate clearly. Must maintain strict confidentially to work related information.
